The information requested for background checks is collected solely for the purpose of conducting a background check in connection with your application to serve as a facilitator or assistant with Capital Christian Homeschool Community of Michigan (CCH).

This process is intended to help ensure the safety and well-being of participants, staff, and the community and is required for all volunteers working with minors.

Providing this information is voluntary; however, failure to provide complete and accurate information may result in the inability to process your background check.

The personal information that you submit, via the provided form, will be used only to:

• Verify your identity
• Conduct authorized background screening

Your information will be:

• Accessed only by members of the CCH board that are responsible for background screening
• Shared only with ICHAT (Internet Criminal History Access Tool) through the state of Michigan
  • To learn more about Privacy Policies and Terms of Use Policy with the state of Michigan, visit https://www.michigan.gov/som/footer/policies
• Handled and stored securely to protect against unauthorized access, disclosure, or misuse until the completion of the background check. Once completed, all forms will be destroyed 

This information will not be used for any purpose unrelated to volunteer screening.

We retain your information only for as long as necessary to complete the background check. After that period, records will be securely destroyed.
